Privacy policy
What we collect, and why
Bone Health CV collects only the information needed to review whether the Strong Bones Blueprint program is a good fit for you and to contact you about it: your name, contact details, and the answers you give in the pre-evaluation form. The bursary application collects your name, contact details and your request — never income details, documents or reasons. The newsletter form collects your name and email, and only sends you updates you asked for.
Who sees it
Your instructor sees your pre-evaluation answers. If you join, Broken Squirrel Wellness administrative staff see only your name, contact details and membership rate, so they can register and bill you. We do not sell your information or share it for other organizations' marketing.
Where it is stored
We use Tally (forms), Notion (internal records, including the screening answers you submit in the pre-evaluation form), an email service provider (newsletter, when we send one) and Lovable (website hosting). Some of these providers may store information outside Canada, including in the United States, where it may be subject to local laws. Access to pre-evaluation answers is limited to your instructor. Clinical records for participants (assessments, progress notes and consent forms) are kept in Jane, the practice-management system used by Broken Squirrel Wellness, under the clinic's retention and privacy practices.
How long we keep it
Clinical records are kept in Jane for the period required by the clinic's retention practices. Pre-evaluation answers for people who do not join are deleted within 90 days of review. For members, they are cleared once your baseline assessment is recorded in Jane. Bursary applications that do not lead to a membership are deleted within 90 days. Other information in Notion and Tally is deleted when it is no longer needed.
Your information, your call
You can ask what information is held about you, ask for it to be corrected or deleted, or unsubscribe from the newsletter at any time. Email info@bonehealthcomox.com.
